Summary:

The Quality Improvement Coordinator will be responsible for the following activities which include standardizing manuals, forms, policies and procedures; and assisting in the efficient clinical review and evaluation of programs. The position involves having direct responsibility for improving systems in order to better support persons served. The position assists with all accreditation activities and preparation for the CARF Survey Process.

This role also involves acting as the lead for all internal and external audits; which are facilitated by the Quality Team. This may include coordination with key external contacts on behalf of the agency, and streamlining inter-departmental and cross functional communication.

The Quality Improvement Coordinator will serve as a leader on the Enterprise Quality Team and will be responsible for overseeing the identification of gaps and recommended changes to clinical practice that impact optimum client care.

This position is also responsible for interfacing with staff and stakeholders represented by all MiSide program pillars.

Duties & Responsibilities:

Responsible for the facilitation and the review of Critical and Sentinel incidents; and the creation of a Root Cause Analysis Report. May be responsible for aspects of death reporting and related information gathering, including medical examiner and other fact-finding documentation.

Co-Facilitates the Behavior Treatment and Utilization Review committees in conjunction with the Medical Director to offer clinical recommendations to staff/teams seeking support for challenging cases; as required by the fund source or to sustain mandated treatment.

Maintain electronic policy system through review and updates; involved in the creation of new policies as necessary in maintaining compliance with regulatory bodies.

Standardize agency forms; archive older versions of forms and documents and upload to the SharePoint for staff use.

Attend Meetings, facilitate and participate in trainings and other project management activities as required.

Lead internal and external audits as one of the main contacts for the Agency, providing information and documentation as necessary and streamline the process for auditors and staff. This may involve audits for all MiSide Program pillars.

Monitoring the results of external audits; verification and facilitation of the corrective action process.

Act as the subject matter expert on Evidence Based Practice and other clinical care methodologies.

Support the Quality team as a leader, guide and support for interns and/or student learners when needed. Provide training where appropriate on Quality Improvement initiatives.

Will conduct investigations for suspected fraud, waste and abuse as part as the agency overall risk reduction program.

May be responsible for daily maintenance and review of incident reports in the Incident Tracker system.
