WORKS IN CLINIC ENVIRONMENT PROVIDING COMMUNITY RESOURCES AND OUTREACH

GENERAL SUMMARY: 

The Social Worker is responsible for creating outcomes for the patient and family by managing complex psychosocial and economic co-morbidities. Provide education and consultation regarding resources, collaborate to develop a discharge plan and facilitate and monitor its implementation. Assist patient/family adjust to and cope with illness, diagnosis, treatment options and prognosis. Through advanced practice skills mobilizes resources to reduce risk, serves as a liaison between the hospital and the community. In addition offers crisis interventions to patient/family.

Social Worker (SW) Primary Responsibilities:

Services provided in Outpatient clinical setting to include:

Completing Social Determinates of Health screening to identify barriers that impact engagement in HIV medical care.

Complete referral(s) to community resource(s) to meet pt need(s).

Documentation of progress notes within EPIC (electronic medical record).

Work within a multi-disciplinary team to meet the needs of population serviced.

Working with diverse populations (LGBTQ+)

Experienced with Microsoft Word / Excel

Foster work relationships with community agencies

Participate in community training opportunities to develop / enhance skills

Insurance Navigation Skills 

Treatment Adherence, Health Education and Risk Reduction Counseling Skills

HIV Counseling, Testing and Referral and Early Intervention Services Trainings must be completed within 1 year of hire
