Description
Under the general supervision of the Public Health Project Leader - HOPWA, the Housing Coordinator within the Housing Opportunities for Persons with AIDS (HOPWA) program is responsible for assisting HOPWA clients with housing related services and activities to promote housing stability with the goal to achieve permanent housing.
Examples of Duties
Conduct the initial assessment, enrollment, and certification of clients, including the collection of eligibility documents.
Perform case management services which involve the assessment for housing, service plan development, monitoring, connection to care, and any other appropriate services or community resources deemed necessary to achieve housing stability.
Assist clients with finding appropriate housing and preparation of lease agreement with rent calculations for all portions of rental subsidy.
Assist clients with obtaining eligibility documents such as identification, social security card, or other documents as required by program.
Facilitate and coordinate supportive activities with other program staff, as outlined in individualized service plans.
Maintain thorough and complete records of participants’ activity and progress toward stated goals.
Partner with other housing providers to understand all available housing options for low-income clients.
Act as liaison between the HOPWA program and other housing options for low-income clients.
Work closely with Community Based Organizations and the Michigan Department of Human Services to improve coordinator of housing opportunities for people living with HIV (PLWH).
Attend city, regional, and state meetings to improve coordination of housing services for PLWH.
Act as liaison between landlords and assigned clients.
Create partnerships with non-traditional housing partners that will increase housing opportunities for PLWH.
Develop relationships with existing and potential landlords to improve access to housing opportunities for PLWH.
Attend housing-related meetings
Facilitate trainings with community non-profits to increase community collaboration and on area community resources.
Facilitate trainings with clients to educate on available community services and resources they may be eligible for.
Partner with other city Departments to ensure optimal housing services are provided to PLWH.
Maintain good working relationships with internal and external customers.
