GENERAL RESPONSIBILITIES 

 

The Social Worker will provide clinical counseling and therapy services to children and families with mental health services including direct counseling services, crisis intervention, and coordination of community services/resources. They will assess clients' needs, develop treatment plans, and provide ongoing support and counseling. They may also collaborate with other healthcare professionals to coordinate care and provide referrals to other services and resources. They are responsible for maintaining accurate client records and ensuring that all services are provided in accordance with ethical and legal standards. 
 

DUTIES AND RESPONSIBILITIES 

 

Conducts comprehensive, culturally sensitive psychosocial assessments, develops care plans, and provides therapy and crisis intervention services to BFDI consumers. 

Participates in team meetings and participates in continually improving care based on results. 

Develops, implements, updates and/or monitors treatment plans for own caseload. 

Assists in maintaining patient pre-authorizations, completing treatment plans in a timely fashion. 

Responsible for accurate, timely communication with the support staff and other clinical staff to maintain high quality services, accurate schedules and clinical records. 

Participates with office on-call schedule. 

Other duties as assigned. 

Works under the supervision of the Project Director to provide high quality mental health services. 

Conducts comprehensive, culturally sensitive bio-psychosocial assessments, develops treatment plans, and provides therapy and crisis intervention services to individuals and families. 

Supplies information and education about mental health issues to individual patients and to groups. 

Documents diagnostic assessment, intervention, and treatment data in medical records. 

Provide support to area school systems with recommendations and attend IFSP, IEP, ARD meetings as needed. 

Ensure all performance improvement activities are managed and performed according to regulatory and accreditation standards, corporate policies, and related measures. 

Provide crisis interventions 

Arrange for services from referral agencies 

Demonstrates competency in practice and knowledge of current standards of practice.  

Maintains credentials and performs current practice expectations within established guidelines. 

Develop and maintain collaborative internal relationships and partnerships with co-workers 

Develop and maintain favorable external relationships with vendors, contractors, and related resources. 

Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional and ethical manner at all times. 

Uphold and ensure compliance and attention to all BFDI policies and procedures as well as the overall mission and values of the organization.
