Behavioral Health Specialist Fellowship Program at Oak Street Health. Oak Street Health (OSH) is a national company providing value-based primary care for older adults in under-resourced communities. We are currently in 25 states with over 240 health centers and are proud to have over 100 licensed clinical social workers providing behavioral healthcare to our most vulnerable seniors. Oak Street Health practices the collaborative care model integrated behavioral health care in a primary care setting. 

A little over two years ago, we launched the inaugural cohort of our innovative Behavioral Health Specialist Fellowship and have had three subsequent cohorts since then. I am excited to share that we are now recruiting for our fifth cohort, which will start in March 2025! We are hiring new MSW graduates for fellowship positions in Texas, Colorado, Tennessee, Pennsylvania,  Rhode Island, New York, Arizona, Alabama, Oklahoma and New Mexico. (Relocation assistance is available). 

Admission Requirements: 

Family, Adult or Adult-Gerontology Social Worker who have already graduated or will graduate from their training program by December 2024. 

Willing to work in the assigned location for the program duration. 

Minimum GPA of 3.0 in MSW program. 

Currently holds or will hold by the fellowship start date, an active, non-probationary state lower level social work license in the state they are applying for.
