Placement Verification Forms (PVFs) Open

For all students currently enrolled in field.

Students must submit their PVF's by 05/13/25 to allow one week for their Field Instructor to review, approve and submit the PVF by 05/20/25. 

All official field-related notifications are sent to the field instructor's UMich email address. To approve the PVF and eventually the student's Educational Agreement, the Field Instructor(s) must sign in using their uniquname and password.

This is a requirement to pass SW515/ SW691.
