Please ensure the PVF is submitted on time as this will ensure the students access to their SS26 Online Educational Agreement (OEA).
This is a requirement to pass SW515/ SW691.
SS26 Placement Verification Forms (PVFs) are Due to Field Instructors for Review & Approval
Students must submit their Placement Verification Forms by Tuesday 5/12/26.
This allows sufficient time for field instructors/task supervisors to make edits and/or approve PVFs by Tuesday 5/19/26.
This is a requirement to pass SW515/ SW691.

For all students currently enrolled in field.
Students must submit their PVF's by Tuesday 05/12/26. This will allow one week for their Field Instructor/Task Supervisor to review, approve and submit the PVF by the final due date of Tuesday 05/19/26.
All official field-related notifications are sent to the field instructor's UMich email address. To approve the PVF and eventually the student's Educational Agreement, the Field Instructor(s) must sign in using their uniqname and password.

Final Online Education Agreement Due (Student/Field Instructor/Task Supervisor Ratings)
We strongly recommend that students submit their OEA to their Field Instructor/Task Supervisor by Friday 11/28/25 to give at least one-week advance notice and allow time for their feedback and submission.
The earliest students can submit their final F25 OEA is on Monday 12/01/25. They will not be accepted prior to this date. The final F25 OEA's are due on Friday 12/05/25.
