About Oak Street Health:
Oak Street Health began with a simple, yet ambitious, mission of Rebuilding Healthcare as it Should Be: personal, equitable, and accountable. We now have a decade-plus track record of providing better care to people who need it the most. Since opening our first center on the north side of Chicago in 2013, Oak Street has since expanded to over 230 centers across 27 states, serving approximately 350,000 patients. We employ over 8,500 individuals, including nearly 900 primary care providers.
About the Fellowship:
The purpose of a Behavioral Health Specialist at Oak Street Health is to provide care management services and to provide evidence-based short-term psychotherapy to our patients with behavioral health needs. The Behavioral Health Specialist is a key resource for Care Teams across a region of Oak Street Health primary clinics. They are responsible for coordinating and supporting behavioral healthcare for our patients, in collaboration with Care Teams and a psychiatric consultant. The Behavioral Health Specialist will assess the needs of patients, make referrals to appropriate behavioral health resources and specialists, and provide short-term counseling and evidence-based treatments as indicated.
Length:
The BHS fellowship is a 12-month paid fellowship. Following completion of the fellowship, Fellows are expected to remain at the centers where they completed the program for 2 years. The length of the program is a total of 3 years.
Structure:
The BHS fellowship is a fellowship program in which the fellow works as a behavioral health specialist providing behavioral health services in an integrated primary care setting. The fellows will build on their knowledge of evidence-based interventions and medications working on an integrated care team (PCP, RN, psych NP, etc.). They will earn clinical supervision hours from a licensed clinical social worker and participate in weekly didactic training to enhance their graduate education knowledge. The fellowship year will end with the presentation of a Capstone Project aimed at bridging the collaborative care model theory and practice.
Support:
Each Fellow will have regular, recurring meetings with their clinical supervisor, BHS Divisional Lead, and Fellowship Faculty. Additionally, time is set weekly for “Fellowship Office Hours” and the fellowship team can be reached out to at any time.
