About the Role:
Location: Metro Detroit Area within driving distance of West Bloomfield Office.
There are some opportunities for periodic travel that will be mutually agreed to as needed.

Summary/Objective:
The primary purpose of the Behavioral Health Case Manager is to team with the Clinical Manager (CLM) in the completion of assessments, planning, coordinating treatment and provide resources for improved wellness and recovery for clients and families. This role requires an in-depth understanding of substance abuse and mental health treatment, ability to professionally engage in front-facing conversations regarding clinical recommendations and next steps to clients and families and execute a high-touch concierge service to ensure that every aspect of a client’s care is comprehensive, integrated, and aligned with their treatment goals. The secondary purpose of the Behavioral Health Case Manager is to obtain knowledge and education regarding options for treatment and services to assist in the development of individualized planning for health, safety, rehabilitation, and recovery for clients and families.

Key Responsibilities:
- Assist the Clinical Manager in the completion of an initial evaluation with client and family to obtain status and history of medical, mental health, family psychosocial dynamics, educational, vocational, and substance use disorder.
- Enter all information from assessment, coordinate in the development of the care plan and update the care plan as needed.
- Update and complete client reports as scheduled.
- Bill and document all case-related activities and ensure billing compliance.
- Create and obtain releases as needed, request medical/mental health records, and review the information related to medical/mental health and substance use disorder findings and treatment.
- Skilled at being clinically assertive in interrupting counterproductive behaviors of clients and families that are not in alignment with the agreed upon primary recommendations while maintaining trust, rapport and respect.
- Exhibit strong interpersonal skills, with the ability to professionally interact with clients and family members independently when the Clinical Manager may be unavailable while maintaining all clinical recommendations and organizational standards.
- Schedule appointments, coordinate follow-up as ordered and/or outlined in the plan of care.
- Build and sustain strong professional relationships with treatment centers, providers and community partners.
- Engage and support in care planning in all team meetings, crisis planning and case reviews.
- Strong ability to multitask and maintain strong time management skills

Required Competencies:
- Ability to perform comprehensive assessments to include a thorough family of origin history.
- Writing care plans to include identification and development of client-centered goals.
- Critical thinking and problem-solving skills for coordination of care.
- Understanding of working with family systems with complex co-occurring presentations, where there is substance use, mental health disorders, and/or process addictions present. This includes an understanding of co-dependency and implementing interventions to interrupt unhealthy patterns in relationships.
- Effective and professional communication skills.
- Effectively navigate various personality traits and disorders.
- Highly organized and display efficient time management skills.
- Demonstrated ability to provide coaching.

Qualifications:
- Master’s degree in social work, counseling or psychology preferred; will consider candidates with a bachelor’s degree in social science and/or certification in alcohol and drug counseling and relevant experience.
- A minimum of 2-5 years in behavioral health, addiction, case management or care coordination.
- Strong understanding of levels of care in mental health or substance use treatment centers.
- Proven ability to navigate complex family systems and collaborate effectively across multidisciplinary teams.
- Skilled in documentation, planning, and follow through in fast-paced, dynamic environments.
- Commitment to providing high-touch, compassionate and holistic client care
- Valid Driver’s License required.
- Ability to acquire increased clinical responsibility and caseload management upon successful completion of training and demonstrated competency.

Compensation:
$75,000 -$100,000 annual salary + benefits based on experience
