Geriatric Home-Based Therapist (Full Time)

Job Summary

  • The Therapist is responsible for assessing, diagnosis and treating individuals and families seeking counseling services, for life stressors, mental health conditions and/or addiction.

Principal Accountabilities 

  • Assessments – Completes a thorough psych-social assessment through collecting and evaluating information in order to provide a clinical diagnosis. All assessments are completed and entered within 5 business days of the intake and/or evaluation dates.
  • Clinical Practice – Treats clients effectively by using Person Centered Planning by engaging clients in the establishment of goals and objectives, progressing clients in treatment and discharging clients upon successful completion of their treatment plan.
  • Paperwork – Maintains client documentation standards by preparing and completing, in a timely manner, all required progress notes, records and reports for all assigned clients.
  • Production – Maintains a caseload in order to conduct and document 60% direct client contact time (on average, 23 billable sessions per week).
  • Compliance – Complies with standards and requirements of all external accrediting/licensing bodies, including attending staff meetings and committee meetings as assigned. Maintain client documentation and recording by preparing and completing all required assessments, treatment plans, progress notes (within 1 days of client contact), records, Discharge Summaries (30 days) and reports for all assigned clients in accordance with agency standards.
  • Ethics – Maintains standard of the profession by following the principles of professional ethics and all applicable codes of ethics.
  • Training – Increases professional and technical knowledge by attending agency in-service programs, educational workshops, reviewing professional publications and establishing personal networks.
  • Record Keeping – Demonstrates awareness of the importance of statistical accountability and fiscal integrity by inputting all time/appointments into Evolv and assuring collection of fees.
  • Networking/Promotion – Promotes the agency by ensuring an understanding of program services available for clients, publicizing activities and accomplishments and outreach with community members as appropriate.
  • Security and Privacy –Adhere to agency and client protection policies by regularly acclimating yourself with the JFS privacy, safety and security policies made available within the employee handbook.
  • Employee Philanthropy –Participate annually in agency and partner related philanthropic activities and campaigns.
  • Mission Dedication –Embody the mission and values of JFS in all work done on behalf of the agency. Utilization of the JFS key strategies will ensure success in this accountability.
  • Misc. – Other duties as assigned by management.

Minimum Qualifications

  • Experience: Minimum two years post-graduate experience in a human service capacity.
  • Education: Minimum of a master’s degree in a field that permits state licensing/registration to provide outpatient counseling/therapy.
  • Certifications or Licenses Required: Advanced licensure in field (LMSW, LLP, LP, LMFT, or LPC) required, CAADC preferred.

  • Technology – Use agency Electronic Health Records to maintain client records and employee schedule, and to document client satisfaction and outcomes.
  • Documentation – Succinctly and accurately document medical necessity for treatment, as well as an individual’s progress or lack of progress in treatment in order to meet insurance, licensing and accreditation standards.

Knowledge

  • Clinical Practice – Be familiar with current Evidenced Based and Promising practices within the therapeutic process, including Person Center Thinking and Motivational Interviewing.
  • Co-occurring Practice – Be familiar with best practices related to providing clinical therapy to individual exhibiting co-occurring mental health and addictions.
  • Public Health – Be familiar with recent local, state and federal policies that affect clinical practice and the delivery of mental health and addiction treatment services.

  • Rapport – Interact in a manner to instill trust and a level of comfort for individual and families in order to build therapeutic rapport so they may engage in the treatment process.
  • Team – Work as an interdisciplinary team with other professionals working with the individual/family within the agency or at other agencies as appropriate.
