    Field Placement Verification Due

    January 23, 2019 - 5:00pm

    All students currently enrolled in field (SW515 or SW691) must turn in the Field Placement Verification form to the Office of Field Instruction. 

    The Field Placement Verification form is not complete until the student submits it AND the field instructor approves it.
